The mandibular third molar (commonly known as a wisdom tooth) is the tooth located distally (away from the midline of the face) from both the mandibular second molars of the mouth with no tooth posterior to it in permanent teeth; they might usually appear between the ages of 16 and 25.[1] They are called wisdom teeth because usually they come in when a person is between age 17 and 21 or older—old enough to have supposedly gained some "wisdom".[2] In deciduous (baby) teeth, there is no mandibular third molar. The function of this molar is similar to that of all molars in regard to grinding being the principal action during mastication (chewing).

Wisdom teeth are commonly extracted when they affect other teeth—this impaction is colloquially known as "coming in sideways".[3] Most adults have four wisdom teeth, but it is possible to have more or fewer. Absence of one or more wisdom teeth is an example of hypodontia. Any extra teeth are referred to as supernumerary teeth.[4][5]

For this tooth, there are great variances among third molars, and a specific description of a third molar will not hold true in all cases. It is important to note that the permanent mandibular molars are not considered to have any teeth that precede it. Despite being named molars, the deciduous molars are followed by permanent premolars.

In the universal system of notation, the permanent mandibular third molars are designated by a number. The right permanent mandibular third molar is known as "32", and the left one is known as "17". In the Palmer notation, a number is used in conjunction with a symbol designating in which quadrant the tooth is found. For this tooth, the left and right third molars would have the same number, "8", but the right one would have the symbol, "┐", over it, while the left one would have, "┌". The international notation has a different numbering system than the previous two, and the right permanent mandibular third molar is known as "48", and the left one is known as "38".

Dentistry is the "evaluation, diagnosis, prevention and/or treatment (nonsurgical, surgical or related procedures) of diseases, disorders and/or conditions of the oral cavity, maxillofacial area and/or the adjacent and associated structures and their impact on the human body". Dentistry is necessary for complete oral health.[1] Those in the practice of dentistry are known as dentists. Other people aiding in oral health service include dental assistants, dental hygienists, dental technicians, and dental therapists.

Dentistry usually encompasses a number of practices related to the oral cavity. The most common treatments involve the dental surgery on the teeth as a treatment for dental caries. Decayed teeth can be filled with dental amalgam, dental composite, dental porcelain and precious or non-precious metals. Oral and maxillofacial surgery is a more specialized form of dental surgery. Dentists can prescribe medication, x-rays, and devices for home or in-office use. Many oral diseases (such as bilateral odontogenic keratocysts) and abnormalities (such as several unerupted teeth) can indicate systemic, neural, or other diseases. Most general practitioners of dentistry perform restorative, prosthetic, endodontic therapy, periodontal therapy, and exodontia, as well as performing examinations. Many general practitioners are comfortable treating complex cases, as well as placing implants and surgically extracting third molars (wisdom teeth). All dentists must achieve a certain degree of skill in various disciplines in order to graduate from dental school and become an accredited dentist.

Prevention

Dentists also encourage prevention of dental cavities through proper hygiene (tooth brushing and flossing), fluoride, and tooth polishing. Recognized but less conventional preventive agents include xylitol, which is bacteriostatic,[2] casein derivatives,[3] and proprietary products such as Cavistat BasicMints.[4]

 

Education and licensing

The Baltimore College of Dental Surgery, the first dental school in the world, opened in Baltimore, Maryland in 1840, and in 1867, Harvard Dental School became the second dental school affiliated with a university.
Studies showed that dentists graduated from different countries[5], or even from different dental schools in one country[6], may have different clinical decisions for the same clinical condition. For example, dentists graduated from Israeli dental schools may recommend more often for the removal of asymptomatic impacted third molar (wisdom teeth) than dentists graduated from Latin-American or Eastern European dental schools.[7]

In England, the 1878 British Dentists Act and 1879 Dentists Register limited the title of "dentist" and "dental surgeon" to qualified and registered practitioners.[8][9] The practice of dentistry in the United Kingdom became fully regulated with the 1921 Dentists Act, which required the registration of anyone practicing dentistry.[10] The British Dental Association, formed in 1880 with Sir John Tomes as president, played a major role in prosecuting dentists practicing illegally.[8] A dentist is a healthcare professional qualified to practice dentistry after graduating with a degree of either Doctor of Dental Surgery (DDS), Doctor of Dental Medicine (DMD), Bachelor of Dentistry (BDent), Bachelor of Dental Science (BDSc), or Bachelor of Dental Surgery/Chirurgiae (BDS) or (BChD) or equivalent. In most western countries, to become a qualified dentist one must usually complete at least 4 years of postgraduate study[citation needed]; within the European Union the education has to be at least 5 years. Dentists usually complete between 5–8 years of post secondary education before practicing. Though not mandatory, many dentists choose to complete an internship or residency focusing on specific aspects of dental care after they have received their dental degree.

A dental emergency is a type of medical emergency involving the teeth or gingiva. Pain involving these parts of the mouth, even when minor, can indicate a severe underlying problem that could worsen with time. It is important to contact a dentist as soon as possible for a professional evaluation to minimize damage to the teeth, gums, and supporting bone. Short-term action can be taken to relieve oral pain, which can be very severe.

Palliatives

Oil of cloves, which contains eugenol, can be used to treat dental pain; a drop can be applied with a cotton swab as a palliative. After wisdom tooth extraction, for example, a condition known as dry socket can develop where nerve endings are exposed to air. A piece of sterile gauze or cotton soaked in oil of cloves may be placed in the socket after careful cleaning with saline to relieve this form of pain.

Over-the-counter topical anesthetics containing active ingredients such as benzocaine or choline salicylate may be applied directly to the gum in order to deaden sensation.

If you prefer a homeopathic approach that does not entail the use of pharmaceutical drugs to managing your dental pain, there are many "home remedies" of varying efficacy for the temporary relief of dental pain.

Analgesics such as aspirin, paracetamol (acetaminophen) and ibuprofen are also commonly used; aspirin and ibuprofen have the additional benefits of being anti-inflammatories. Ice and/or heat are also frequently applied. A dentist may prescribe an anti-inflammatory corticosteroid such as Dexameth for pain relief prior to treatment. [1]

A dental extraction (also referred to as exodontia) is the removal of a tooth from the mouth. Extractions are performed for a wide variety of reasons, including tooth decay that has destroyed enough tooth structure to prevent restoration. Extractions of impacted or problematic wisdom teeth are routinely performed, as are extractions of some permanent teeth to make space for orthodontic treatment.

 

History

Historically, dental extractions have been used to treat a variety of illnesses, as well as a method of torture to obtain forced confessions. Before the discovery of antibiotics, chronic tooth infections were often linked to a variety of health problems, and therefore removal of a diseased tooth was a common treatment for various medical conditions. Instruments used for dental extractions date back several centuries. In the 14th century, Guy de Chauliac invented the dental pelican, which was used through the late 18th century. The pelican was replaced by the dental key which, in turn, was replaced by modern forceps in the 20th century. As dental extractions can vary tremendously in difficulty, depending on the patient and the tooth, a wide variety of instruments exist to address specific situations.

 

Reasons for tooth extraction

The most common reason for extraction is tooth damage due to breakage or decay. There are additional reasons for tooth extraction:

  • Severe tooth decay or infection. Despite the reduction in world-wide prevalence of dental caries, still it is the most common reason for extraction of (non-third molar) teeth with up to two thirds of extractions.[1]
  • Extra teeth which are blocking other teeth from coming in.
  • Severe gum disease which may affect the supporting tissues and bone structures of teeth.
  • In preparation for orthodontic treatment (braces)
  • Teeth in the fracture line
  • Fractured teeth.
  • Insufficient space for wisdom teeth (impacted third molars). Although many dentists remove asymptomatic impacted third molars,[2][3] American as well as British Health Authorities recommended against this routine procedure, unless there are evidences for disease in the impacted tooth or the near environment.[4]
  • Receiving radiation to the head and neck may require extraction of teeth in the field of radiation.
