Invisalign

From Wikipedia, the free encyclopedia

Invisalign Logo

Invisalign is a series of clear, removable teeth aligners that both orthodontists and dentists use as an alternative to traditional metal dental braces. As of April 2008, more than 730,000 patients have completed or are currently in treatment.[1]

Invisalign is designed, manufactured, and marketed by Santa Clara-based medical-device company Align Technology, Inc. Align says that over 35,790 doctors are trained to provide Invisalign treatment in the U.S., with 48,130 doctors worldwide.[1] As of January 29, 2008, Align Technology has 1,307 employees worldwide, and has manufactured more than 32 million aligners. The company has 133 patents[citation needed].

Align Technology was in a legal battle with the makers of a competing product, OrthoClear, from early 2005 until September, 2006.[2] Zia Chisti, one of the founders of Align, had started OrthoClear to compete against Invisalign. In a complaint filed with the United States International Trade Commission (ITC) on January 11, 2006, Align alleged that OrthoClear utilized Align's trade secrets and infringed twelve Align patents, comprising more than 200 patent claims, in the production of OrthoClear aligners at a facility in Lahore, Pakistan. On September 27, 2006, Align Technology settled its litigation with OrthoClear. OrthoClear has stopped accepting new cases and discontinued its aligner business worldwide. Align acquired all disputed intellectual property. Contrary to some reports, Align did not purchase OrthoClear.[2]

Advantages and disadvantages

Advantages

The most obvious advantage of the treatment is cosmetic: the aligners are completely transparent, therefore far more difficult to detect than traditional wire and bracket braces. This makes the method particularly popular among adults who want to straighten their teeth without the look of traditional metal braces, which are commonly worn by children and adolescents. In addition, the aligners are marketed as being more comfortable than braces.[3] Due to the removable nature of the device, food can be consumed without the encumbrance of metallic braces.

Clinically, aligners avoid many of the side effects of traditional fixed appliances,[4] for example the effects on the gums and supporting tissues.[5] Fixed appliances are known to cause the roots of teeth to shorten for most patients,[6] and demineralisation or tooth decay occurs in up to 50% of patients[7] because they cannot be removed for eating and cleaning, and because they prevent accurate x-rays from being taken. Patients "graduate" to a new set of aligners in their treatment series approximately every two weeks. The aligners give less force per week and less pain than do fixed appliances (traditional metal braces). Fixed appliances are adjusted approximately every six weeks and apply greater forces.[8]

Aligners should be removed to eat, drink, to clean the teeth, or to have them checked by the clinician. (It is acceptable to wear aligners while drinking water.) Computerized treatment planning is compulsory as part of the Invisalign protocol. As with other forms of orthodontic treatments that incorporate a computerized plan, this allows the prospective patient to review the projected smile design, learn how long the treatment is likely to take, compare different plans, and make a more educated decision about whether or not to use Invisalign.

Invisalign treatments have been claimed to be quicker than traditional orthodontics. A large-scale study of 408 patients with traditional appliances in Indiana took an average of 35.92 months with a maximum of 96 months,[9] while Invisalign is taking between 12–18 months.[4][10] In a much smaller study[11] Invisalign was shown to be faster and achieve straighter teeth than alternatives but relapsed to ultimately get similar results to the traditional appliances examined. The study was considered by the authors, however, to be too small for many conclusions to be statistically significant. Furthermore, this general concept that Invisalign is faster has been challenged by the Invisalign review which points out that there are other brace appliance systems that take half the time, for example by incorporating surgery or temporary implants that insert into the patient's bone, to accelerate the procedure.[12]

Disadvantages

The product also has potential disadvantages: the very fact that the aligners are removable means they are not continually correcting the teeth. Unlike traditional fixed braces, they are largely dependent on a patient's habits and their consistency in wearing the aligners. The success of the Invisalign aligners is based on a patient's commitment to wear the aligners for a minimum of 20–22 hours per day, only removing them when they are eating, drinking, or brushing their teeth.

According to Wikipedia [1]: Orthodontics is a specialty of dentistry that is concerned with the study and treatment of malocclusions (improper bites), which may be a result of tooth irregularity, disproportionate jaw relationships, or both. The word comes from the Greek words ortho meaning straight and odons meaning tooth. Orthodontic treatment can focus on dental displacement only, or can deal with the control and modification of facial growth. In the latter case it is better defined as "dentofacial orthopedics". Orthodontic treatment can be carried out for purely aesthetic reasons with regards to improving the general appearance of patients' teeth. However, there are Orthodontists, who work on reconstructing the entire face, rather than focusing exclusively on teeth. Nonetheless, treatment is most often prescribed for practical reasons such as providing the patient with a functionally improved bite (occlusion).

Offices in In dentistry, a veneer is a thin layer of restorative material placed over a tooth surface, either to improve the aesthetics of a tooth, or to protect a damaged tooth surface. There are two main types of material used to fabricate a veneer, composite and porcelain. A composite veneer may be directly placed (built-up in the mouth), or indirectly fabricated by a dental technician in a dental laboratory, and later bonded to the tooth, typically using a resin cement such as Panavia. In contrast, a porcelain veneer may only be indirectly fabricated.

Tooth bleaching, also known as tooth whitening, is a common procedure in general dentistry but most especially in the field of cosmetic dentistry. Many people consider white teeth to be an attractive feature of a smile. A child's deciduous teeth are generally whiter than the adult teeth that follow. As a person ages the adult teeth often become darker due to changes in the mineral structure of the tooth, as the enamel becomes less porous. Teeth can also become stained by bacterial pigments, foodstuffs and tobacco.

The procedure to bleach teeth uses oxidizing agents such as hydrogen peroxide or carbamide peroxide to lighten the shade of the tooth. The oxidizing agent penetrates the porosities in the rod-like crystal structure of enamel and oxidizes interprismatic stain deposits; over a period of time, the dentin layer, lying underneath the enamel, is also bleached. The effects of bleaching can last for several months, but may be shortened by cigarette smoking, and tea and coffee consumption.

So, if your anything but passionate about your smile, schedule a complimentary appointment at a certified Teeth Whitening Houston office to learn more how this cosmetic veneering procedure may be right for you.

Veneer (dentistry)

From Wikipedia, the free encyclopedia

(Redirected from Porcelain veneers)

In dentistry, a veneer is a thin layer of restorative material placed over a tooth surface, either to improve the aesthetics of a tooth, or to protect a damaged tooth surface.

History

Veneers were invented by a California dentist named Charles Pincus [1]. At the time, they fell off in a very short time as they were held on by denture adhesive. They were, however, useful for temporarily changing the appearance of movie actors'/actress's teeth.

Research started in 1982 by Simonsen and Calamia [2] revealed that porcelain could be etched with hydrofluoric acid and bond strengths could be achieved between composite resins and porcelain that were predicted to be able to hold porcelain veneers on to the surface of a tooth permanently. This was confirmed by Calamia [3] in an article describing a technique for fabrication, and placement of Etched Bonded Porcelain Veneers using a refractory model technique and Horn [4] describing a platinum foil technique for veneer fabrication. Additional articles have proven the long term reliability of this technique. [5][6][7][8][9][10][11][12][13]

Today, with improved cements and bonding agents, they typically last 10-30 years. But patients who receive veneers should understand that they may only last 10-15 years and then may have to be replaced. This can be very expensive since porcelain veneers cost around $1,000 each in 2006 prices. They are said to be somewhat more durable and less likely to stain than veneers made of composite.

Usage

The advantages of using a veneer to restore a tooth are numerous. Very pleasing aesthetics can be obtained, with minimal tooth preparation (i.e. drilling). Many veneer procedures can be done with the use of little or no local anesthetic. Traditionally, a reduction of approximately 0.5 mm is required for a porcelain veneer. Composite veneers are becoming more popular as they are easy to repair, whereas porcelain veneers have potential to fracture. It can be very difficult to match the shade of an individual veneer to the remaining teeth, hence placing several veneers is common.

There are some veneers which do not require any drilling in order to remove tooth structure. Instead, these veneers are constructed to be placed on top of teeth. As a result, treatment is less invasive and may be less time-consuming. On the other hand, since the teeth are not reduced in size the veneers may appear too large or bulky unless the material used is extremely thin. Therefore, the success for these veneers is best when limited to specific cases.

Veneers may be used cosmetically to resurface teeth such as to make them appear straighter and possess a more aesthetically pleasing alignment. This may be a quick way to improve the appearance of malposed teeth without need to use orthodontics. However, the amount of malposition of teeth may be such that veneers alone may not be enough to correct the aesthetic imbalance. Instead, orthodontics would need to be used, or orthodontics combined with veneers. The dentist who places veneers must be careful since veneers could increase the thickness of the front face of the teeth. If the teeth are too thick on the face they may appear to stand out and push out the lips. The effect may be enough to give the patient a full or donkey appearance when the lips are closed. Veneers must also be created such that the patient bites into them with minimal force. Otherwise, they may chip off. So, patients whose lower jaw protrudes out farther than their upper jaw, otherwise known as a class III bite, may not be good candidates for veneers because the teeth of the lower jaw may bite into the teeth of the upper jaw such as to dislodge the veneers.

Indications

Veneers are an important tool for the cosmetic dentist. A dentist may use one veneer to restore a single tooth that may have been fractured or discolored, or multiple teeth to create a "Hollywood" type of makeover. Many people have small teeth resulting in spaces that may not be easily closed by orthodontics. Some people have worn away the edges of their teeth resulting in an unpleasing and prematurely aged appearance, while others may have malpositioned teeth that appear crooked. Multiple veneers can close these spaces, lengthen teeth that have been shortened by wear, provide a uniform color, shape, and symmetry, and instantly straighten the teeth's appearance. While orthodontics may be the preferred method for straightening most teeth, some teeth may remain unattractive with flaws in their teeth may require veneers to enhance them anyway. These people would greatly benefit from veneers and may not need to go through braces at all. A cosmetic dentist may be able to show you how the end result may look before you begin the procedure through the aid of a mockup. This is done by molding and shaping tooth colored material to the teeth, or can be done with computer imagery.

Alternatives

In the past, the only way to correct teeth imperfections was to cover the tooth with a crown. Today, in most of the cases there are 3 alternatives : crown, composite resin bonding or porcelain veneer. Each treatment has its pros and cons but, if your teeth are healthy without other dental problems and you want the best cosmetic result, porcelain veneers are the preferred solution despite their higher cost.

According to Wikipedia [1]: Dental braces (also known as orthodontic braces) are a device used in orthodontics to align teeth and their position with regard to a person's bite. They are often used to correct malocclusions such as underbites, overbites, cross bites and open bites, or crooked teeth and various other flaws of teeth and jaws, whether cosmetic or structural. Orthodontic braces are often used in conjunction with other orthodontic appliances to widen the palate or jaws or otherwise shape the teeth and jaws. They are mainly used on children and teenagers, however adults can also use them.
